What is whiplash?

Whiplash is a neck injury caused by a rapid back-and-forth motion of the head — most often in a rear-end car accident. The sudden force strains the muscles, ligaments, and soft tissues of the neck, and can also affect the discs and nerves. It is the most common injury after a collision.

When the ligaments between the vertebrae are the tissue that gives way, the injury is specifically a cervical sprain, which heals more slowly than muscle because ligament has a limited blood supply.

Whiplash symptoms — often delayed

Symptoms can appear hours or even days after the accident. They include:

Headaches, often starting at the base of the skull — see post-traumatic headache

• Neck pain and stiffness
• Reduced range of motion in the neck
• Headaches, often starting at the base of the skull
• Pain spreading to the shoulders or upper back
• Dizziness, fatigue, or difficulty concentrating
• Tingling or numbness in the arms

Because symptoms are frequently delayed, it’s important to be evaluated even if you felt fine at the scene.

How whiplash is diagnosed

Diagnosis starts with a physician examination of your neck’s range of motion, tenderness, and neurological signs. Imaging — such as on-site digital X-ray, and MRI when indicated — helps rule out fractures and disc injuries and confirm the diagnosis.

Most of what an exam finds is a cervical strain — microtears in the neck muscles and their tendon attachments, which do not appear on a plain X-ray and are sometimes dismissed at the ER as a result.

Whiplash treatment

Most whiplash is treated without surgery. A typical plan combines:

• Physical therapy to restore range of motion and strength
• Chiropractic care for the spine and soft tissue
• Pain management with non-surgical approaches
• Guidance on activity, posture, and a gradual return to normal movement
